Accidental Death & Dismemberment

Accidental Death & Dismemberment (AD&D) is a plan that pays a benefit if you lose your life, limbs, eyes, speech or hearing due to an accident.

You can enroll for individual or family coverage. Family coverage includes your spouse and any dependent children between ages 1 and 19 years (or up to age 23 if attending school full time). There are three types of family payouts:

  1. If you and your spouse have no children, the amount paid to you if your spouse died in an accident (or paid to them if the accident caused dismemberment, but not death) would be 50% of your coverage.
  2. If you and your spouse have children, the amount paid to you if your spouse died in an accident (or paid to them if the accident caused dismemberment, but not death) would be 40% of your coverage, and the amount paid to you if your child died in an accident (or paid to them if the accident caused dismemberment, but not death) would be 10% of your coverage.
  3. If you have children, but do not have a spouse, the amount paid to you if your child died in an accident (or paid to them if the accident caused dismemberment, but not death) would be 15% of your coverage.

How to Enroll

Full-time regular staff are eligible for AD&D coverage. You enroll as a new hire by using My VUMC Benefits.

  • You can elect individual or family coverage in increments of $10,000 up to a maximum of 10 times your Annual Base Benefits Rate or $500,000, whichever is less.
  • If you choose family coverage, your spouse and children are covered at a percentage of the amount of coverage that you elect.
  • Changes can be made during annual Open Enrollment or if you experience a qualifying life change.

How to File a Claim

Contact the Employee Service Center.

Please have this information ready when calling the Employee Service Center:

  • Name of deceased
  • Employee ID (if deceased if an employee) or SSN (if deceased is not an employee)
  • Date of birth
  • Date of death
  • Any known beneficiaries (not required)
  • Address (if different than the employee's)
  • Name and relationship of the person calling in (if not the employee)
  • Contact number for the person calling in

Travel Assistance

If you purchase $10,000 or more of AD&D insurance, you are eligible for MetLife's Travel Assistance. The payroll premium for $10,000 of AD&D coverage is $.14 (14 cents) per month. Participants have access to assistance when faced with an emergency while traveling internationally or domestically when more than 100 miles away from home.

MetLife provides worldwide medical, travel, concierge, legal and financial assistance services through Travel Assistance which includes:

  • medical referrals and appointments with English speaking doctors, dentists and specialists (over 600,000 pre-qualified providers in more than 238 countries)
  • hospital admission validation
  • emergency evacuation
  • air and ground ambulance service
  • trained multilingual personnel who can advise and assist you in a travel emergency
  • repatriation benefit
  • return of mortal remains
  • lost document and luggage assistance

As long as the service provided is covered, you will not have to pay the provider upfront and then look for reimbursement.

In most instances the employee will be relying on AXA to help arrange the services and AXA and the vendor work out the payment arrangements. The Maximum Benefit Amount is 100% of covered expenses up to $200,000 per Eligible Person per trip. Learn more by reading the MetLife Travel Assistance brochure.

Beneficiaries

To designate or change your AD&D beneficiaries, go to My VUMC Benefits. Select the My Beneficiaries dropdown link from the Home button.
