Medical Center Card Services - Visiting Students

To receive a card, visiting students and working professionals must obtain a signed Vanderbilt Card Authorization Form from their VUMC contact and submit it to the Medical Center Card Services office at Medical Center North.

The VUMC contact is the individual who is coordinating and/or sponsoring the student's or working professional's visit. If the VUMC contact has submitted documentation to us prior to the visit, then the visiting student or working professional can complete the Student Form in the Card Services office.

Card Fee Amount Required Documentation
New Card No Fee Photo ID, Vanderbilt Card Authorization Form signed by appropriate person
Worn Out Card No Fee Old Card, No Form unless card is illegible. If illegible the student must fill out the Vanderbilt Card Authorization Form
Lost/Damaged Card $20.00 - No Refunds Photo ID, Vanderbilt Card Authorization Form signed by student. -- Cash, Check or Credit Card
Stolen Card No Fee Photo ID, Police Report, Vanderbilt Card Authorization Form signed by student; if no police report - $20.00 Fee

Hours*

Effective Aug. 8, the Card Office hours are:

  • Monday-Friday: 8:30 a.m. - 4 p.m.
  • Monday: closed for lunch from 1:30 - 2 p.m.
  • Tuesday-Friday: closed for lunch from 12:30 - 1:30 p.m.

*The office may close occasionally throughout the week due to system upgrades and staff meetings. Please call to verify operational hours on the day you plan to visit.
