Request for Accommodations

Requests for reasonable accommodations for medical or religious purposes are all managed and facilitated through the Employee and Labor Relations team. Once a request for an accommodation is made, Employee and Labor Relations will engage with the employee and department through an interactive dialogue to determine accommodation outcome.

Medical Requests for Accommodations Process

If you believe you need a medical accommodation to complete the essential functions of your job, please contact the Employee and Labor Relations team at 615.343.4759 or employeerelations.vumc@vanderbilt.edu. You will be asked to provide information about your disability and what you are requesting to accommodate your disability. You need to complete the Reasonable Medical Accommodation Request Intake Form and return to Employee and Labor Relations by mail or email to determine if you meet the definition of an individual with a disability. Once Employee and Labor Relations has determined that you are qualified, the following interactive process steps will occur:

  • Consultation with you and your supervisor.
  • Identification of potential accommodations that will allow you to perform the essential functions of your job.
  • Understanding and clarification on what accommodations (if any) have been decided.

When seeking medical accommodations, employees should keep in mind:

  • Accommodations can only be implemented from the time of the request forward and are not retroactive.
  • The determination of a need for medical accommodations cannot undo previous consequences of poor work performance.
  • All employees must be able to perform the essential functions of their job.
  • Determination of reasonable accommodations is developed based on the employee's individual disability and the specific function of the job.
  • Information regarding an individual's disability is considered confidential.

Please note: in order to determine eligibility, most employees will be required to have a qualified healthcare provider complete the Reasonable Accommodation Medical Provider Form. If this is required, employees must work closely with their provider to return the completed form so the accommodation discussion can occur. Delays in returning the Medical Provider Form will cause delays in moving through the accommodation process and the timeliness of VUMC's response to the request.

Religious Request for Accommodation Process

If you believe you need a religious accommodation as a current or prospective employee, please contact the Employee and Labor Relations team at 615.343.4759 or employeerelations.vumc@vanderbilt.edu. You will need to submit the Religious Reasonable Accommodation Request Form to Employee and Labor Relations by mail or email. Once the completed form has been received, Employee and Labor Relations will follow the following interactive process:

  • For current employees; consultation with you and your supervisor.
  • For prospective employees; consultation with you and the potential supervisor for the position applied for.
  • Identification of potential accommodations that may allow you to accommodate your request.
  • Understanding and clarification on what accommodations (if any) have been decided.

When seeking religious accommodations, employees should keep in mind:

  • Accommodations can only be implemented from the time of the request forward and are not retroactive.
  • Submitting a request for accommodation does not automatically mean it will be approved.
  • Requests for religious accommodations must be reviewed annually.
